Rhinoplasty can be done through open or closed surgery. Though the closed method is preferred due to its lack of a visible scar, the open method allows the surgeon to make more significant alterations.
During an open rhinoplasty, the surgeon makes a small incision across the columella, which is the strip of skin between the nostrils. Dr. Pollei then lifts the skin up and away from the tip of the nose, gaining full access to the bone and cartilage that shapes the nose. The open method is used to build up or reduce the bridge. Once the alterations are complete, the surgeon will re-drape the skin over the new structure. The patient will have a small scar on the columella that fades with time.
During a closed rhinoplasty, the surgeon makes all incisions within the nose, avoiding any visible scarring. The closed method is used when the patient wants to change the overall nose size and address breathing problems. Once the incisions are made, the surgeon separates the soft tissues from the bone and cartilage.
If the patient’s nose is being thinned or reduced in size, the surgeon will fracture the nasal bones to allow reduction. When increasing nose size, the surgeon will use synthetic material, cartilage, or bone from the patient’s ears or ribs.
